I live in kennels since July and I just can not get used to the idea that my life should spend behind these bars. Via l'aggettivo «permanente » per connotare lo stato vegetativo che può subentrare al coma dopo patologie improvvise o incidenti, come era accaduto a Eluana Englaro: quei pazienti vanno invece considerati persone con «gravissima disabilità». Lo raccomanda il gruppo di lavoro ad hoc presieduto dal sottosegretario alla Salute, Eugenia Roccella, nel documento finale presentato ieri al ministero, insieme al libro bianco redatto da alcune associazioni dei familiari.
Nessuna confusione tra stato vegetativo, malattia terminale e morte cerebrale: il concetto di irreversibilità va bandito, così come la locuzione «staccare la spina ». Perché sono troppe, secondo gli esperti, le incognite scientifiche sull'attività della coscienza e troppi, fino al 42%, gli errori diagnostici. E perché non c'ècertezza assoluta che il paziente «non possa provare qualche forma di sofferenza». «Sono vivi, vanno curati», ha detto Roccella.
Gli effetti del cambiamento terminologico non sono da poco: alimentazione e idratazione assistite sono «elementi fondamentali dell'assistenza» (la loro sospensione, come avvenne con Eluana, è considerata eutanasia) e la persona in stato vegetativo ha diritto ad antidolorifici ogni volta che «vengano diagnosticate verosimili fonti di dolore» (il gruppo di lavoro si riserva l'approfondimento delle modalità di percezione dello stimolo della sete). Soprattutto, «al pari degli altri individui con gravissime patologie croniche questa persona può essere preferibilmente accolta a domicilio o, quando ciò risulta impossibile, può essere trasferita in strutture a carattere non prettamente sanitario».
Prendendo atto dell'eterogeneità e criticità dei servizi nelle regioni, il gruppo di lavoro propone la creazione di un «sistema esperto a rete integrata di percorsi dal coma al domicilio». Ma come potrebbero le famiglie sostenere il peso dell'assistenza? Un team riabilitativo della Asl dovrà valutare la capacità della famiglia di poter gestire la situazione a casa, assicurandole sostegno.
